To schedule a time for Texas Elite Eye Care to perform mobile eye exams at your nursing facility, please contact us at your earliest convenience at wecare@texaseliteeyecare.com. We require the skilled nursing home to complete the attached Contract Proposal to begin the process. One we get your contract and secure a date, your facility will need a face sheet for each resident indicating their consent for an eye exam along with their insurance information. Our team will coordinate with you to ensure a seamless and convenient experience for your residents, providing them with the high-quality eye care they deserve.
